What is your new Primary Care Patient procedure?
To become a new primary care patient, you must verify that your insurance participates with our office, and change your PCP to Dr. Luis Melgar. If your PCP listing is not Dr. Melgar when you come in for your appointment, you may be asked to reschedule.

You are asked to arrive 15 minutes early to complete any paper work (Consent for Treatment, HIPAA, Ect.)
What is your New Specialty Patient (Asthma, Allergy, Immunology)  procedure?
To become a new Asthma, Allergy, Immunology (specialty) patient, you must verify that your insurance participates with our office. You must have your PCP send us records

What is your late policy?
If you are more than 15Â minutes late for your appointment you may be rescheduled.
I have chronic pain, will you prescribe me pain medications?
Our providers feel that chronic pain is best managed by a specialist, therefore, we DO NOT prescribe pain medications for chronic pain. We will refer patients to a pain specialist for treatment of chronic pain.
